How to Make a Consultation Appointment:
If you have been advised by your dentist or GP that you need to see one of our specialists, please contact our reception team on 43231277 and they will be ready to help you arrange a convenient time. Our consultation rooms are located in Gosford on the Central Coast; Mona Vale on the Northern Beaches; Bondi Junction, Haymarket and Macquarie St in Sydney. Our staff will assist you with the days and times available at these different locations.
Referrals:
As a specialist practice we will need you to bring a referral letter from your dentist, orthodontist, GP or hospital. Not only will this advise the doctors exactly what you are seeing them for, it is required for any claiming you may be entitled to through Medicare or your private health fund. Your referring doctor may have forward this referral to us via fax or email prior to your appointment, or it may have been given to you to bring with you to the consultation.
XRays:
For any dental consultation you will also need to bring a current PA or OPG x-ray. Your dentist or orthodontist may forward this to us via email prior to your appointment, or it may be given to you to bring to consultation.
If you have been discharged from a hospital with possible facial fractures, please ensure that you have been given the disc of X-rays, along with your discharge summary letter, to bring with you to the consultation.
What to bring:
• Your Medicare or DVA card - this is important for your claiming purposes
• Your Private Health fund card – again, this is important for your claiming purposes
• X-ray – if you have been given your x-ray, please make sure that you bring it with you to your consultation, as
the surgeon will not be able to complete your assessment and you may need to be re-appointed.)
• Your referral letter if the doctor or dentist has not already sent it through.
• A list of your current medication – medications need to be taken in to consideration when planning your
treatment
At your appointment:
For your convenience you are welcome to print and complete the attached ‘Confidential Patient Information’ form prior to your consultation and bring it with you on the day of your appointment. On arrival, you will be asked to complete an information form which will ask for your personal details such as address, contact numbers, current dentist and GP, Medicare and health fund details, previous surgeries, illnesses and current medications.
The doctor will then discuss with you the reason for referral and his suggested treatment options. You will be given a quotation for expected costs & the associated item numbers.
Please be advised that no surgical treatment will be undertaken on the day.
If you elect to undergo surgery, hospital admission and consent forms can be completed at this time, or you may ring back to confirm arrangements at a later date.
Payment:
Payment for the consultation is required at the time of the appointment.
For dental consultations, a rebate is claimable through a private health fund (dental extras cover) only.
Note: There is NO Medicare contribution towards this appointment. HICAPS is available in Gosford rooms only.
Medical consultations attract a Medicare rebate. Our staff can assist with lodging a claim with Medicare online on your behalf.
